- On the first page, on the right side, you will see a button you can use to change the language to your preference. Your options are English and Spanish.
- You will then be prompted to enter a few details on the webpage. These details are Charley’s store number, the date of your visit, the time of your visit, and the type of the visit, was it Dine In or Take Out. All this information can be found on the receipt you were issued after you made your purchase.
- You will also be required to answer a question on whether that visit was the first one you have ever made at any of Charley’s restaurants. The choices are Yes/No, choose the one that applies to you.
- Next, click the “Start” button to begin the survey.
- You will see a typical multi-choice questionnaire with survey questions concerning your visit to the store. Try and recall your experience to answer the questions as accurately and honestly as possible.
